    JOINT AFFIDAVIT OF SUPPLEMENTAL REPORT


    WE, Spouses ERNESTO ARAGON and AIDA DICHOSO, both of
    legal age, Filipinos, and residents Brgy. Chitongco, Mondragon, Northern
    Samar, after having been duly sworn to accordance with law, hereby
    depose and states:

    1. That we were married on FERUARY 22, 1980, at the OFFICE


    OF THE MUNICIPAL MAYOR, MONDRAGON, NORTHERN,
    SAMAR, which fact of our marriage was duly registered under
    Registry No. 9;

    2. That when a copy of our Certificate of Marriage was secured it


    was discovered that our DATE OF MARRIAGE which is
    “FEBRUARY 22, 1980” was not indicated in the document;

    3. That the foregoing missing data was a result of the


    misunderstanding between the informant and the person who
    caused the recordings of the same, hence, not intentional;

    4. That we execute this Joint Affidavit in order to attest to the


    truthfulness and veracity of the foregoing and in order to support
    our Application for the necessary correction and supplement of
    data/entry in our Certificate of Marriage particularly in our DATE
    OF MARRIAGE which is FEBRUARY 22, 1980” and for
    whatever legal purpose this may serve;

    In witness whereof, we have hereunto set our hands this 4th day of
    March 2014 at Catarman, Northern Samar.
