Job Title Specialist Registrar in Vascular Surgery

Context
The London Cardiovascular Project approved the case for change for the way that
specialist Vascular services are provided as part of a reorganisation of Cardiac and
Stroke services across London. Barts Health NHS Trust became the designated
specialist Vascular unit in North and East London.

The hospitals have been caring for the people of East London and the City for centuries and
been at the forefront of clinical care, teaching and research since the establishment of the
country’s first medical school here in 1785. Barts Health NHS Trust is one of Britain’s
leading healthcare providers and the largest trust in the NHS. It was created on 1 April 2012
by bringing together three trusts: Barts and The London NHS Trust, Newham University
Hospital NHS Trust and Whipps Cross University Hospital NHS Trust. The new trust has a
turnover of approximately £1.1 billion and approximately 15,000 employees. Annually the
Trust sees 135,000 attendances at our Emergency Department and Minor Injuries Unit,
380,000 outpatients and 86,000 inpatients at our hospitals.

The hospitals offer a full portfolio of services that serve the needs of the local community,
and are home to some of Britain’s leading specialist centres including cancer, cardiac,
trauma and emergency care. Barts Health also has one of the UK’s busiest children’s
hospitals and internationally renowned surgical facilities.
well as outpatients, emergency services is provided 24/7 by expert multi-disciplinary teams
and because there are larger pools of expert staff, more services are provided for patients
seven days a week and for more hours of the day, providing prompt access to treatment in
all departments and helping reduce waits and cancellations. Patients will also be able to take
part in a wider range of clinical trials, knowing they will be treated by teams working at the
forefront of innovation.

The Vascular surgery unit is also, crucially, part of a wider network of vascular provision
delivered by Barts Health, including services at Newham, The Royal London, Whipps Cross,
St. Barts and Homerton. The unit specialists will work with these and other hospitals in the
area to provide a comprehensive network of care for patients, and ensure that the majority of
care is provided as close to home as possible.

Job Purpose

Barts Health is looking for a fixed term (6 months) Vascular Specialist Registrar who will be
predominantly based at the Royal London Hospital site with responsibilities across the Trust
hospitals sites.

The post would suit a surgeon who has completed core surgical training, or at any stage of
higher training, who wishes to continue to maintain and gain experience in vascular surgery.
The successful applicant will primarily ward based with access to operating theatres and will
not participate on the on call rota and will not have any general surgery commitment.

The successful candidate will be required to adhere to the Trust’s policy on Maintaining
Medical Excellence and to be committed to maintaining their standard of performance by
keeping their knowledge and skills up to date.

Clinical Governance
There is a robust clinical governance programme with a monthly morbidity and mortality
meeting. The post holders are also expected to partake in at least one audit project related
to their subspecialty attachment. There is excellent support for this through the audit.
Study, Training and Teaching
Appointees will be expected to have completed foundation and core surgical training and
have passed the MRCS (or equivalent) examination. At present, these posts do not carry
deanery accreditation for the purposes of surgical training, though it is expected that they will
provide training that will be of value to trainee surgeons.

Research
Extensive research facilities exist at Barts Health NHS Trust and will be actively encouraged
within the department and in conjunction with other departments of the hospital and
university where there is an unusually wide range of research activity.

The post holder is expected to undertake the appropriate management responsibilities, and

Infection Prevention and Control Roles and Responsibilities


 To adhere to Trust policy and infection control principles and standards to minimise
patient risk and ensure high quality patient care
 Undertake appropriate training and practice to ensure you (and your teams) have the
right skills and are competent
 Are responsible for keeping the environment clutter free, clean and raising issues of
concern in the interest of staff and patient safety

Salary and Conditions of Service


Your salary will be at the appropriate point on the scale, in keeping with your experience and
your work intensity.

The post is subject to the Terms and Conditions of Service of Hospital Medical and Dental
Staff (England and Wales), to the General Whitley Council Conditions of Service, and to the
National Health Service Pension Regulations.
Due to the nature of the work in this post, it is exempt from the provision of section 4 (2) of
the Rehabilitation of Offenders Act 1974, by virtue of the Rehabilitation of Offenders Act
1974 (Exemption Order 1975).

Applicants are therefore not entitled to withhold information about convictions, including
those, which for other purposes are “spent”, under the provisions of the Act, and are
required to disclose convictions including those pending, to the Trust. Failure to disclose
such information may result in dismissal or disciplinary action.

Hours of Work
Working hours will be Monday to Friday 08:00 – 17:00 with no on call.

Barts Health NHS Trust reserves the right to change the parameters of the Standard working
week, the number of hours additional duty you are required to undertake and the area in
which you are required to undertake it, in the light of the requirements of the service. Barts
Health NHS Trust will endeavour to give as much advance notice as possible of such
change. In the event of a change in the number of additional duty hours you are required to
undertake your pay will be adjusted appropriately.

Leave
Annual leave will be taken as per the current guidelines (currently 32 days). Notice must be
taken of the Trust and Departmental Guidelines regarding leave must be adhered to with 6
weeks’ notice given.

The above is an outline only. It is not exhaustive may be altered from time to time in
accordance with the needs of the service. The post holder will be required to be co-operative
and flexible in accordance with the needs of the service.

The post holder will be required to work across the Trust at any time throughout the duration
of his/her contract, which may entail travel and working at different hospitals.

Safeguarding adults and children


Employees must be aware of the responsibilities placed on them to maintain the wellbeing
and protection of vulnerable children and adults. If employees have reason for concern that
a patient is 'at risk' they should escalate this to an appropriate person i.e. line manager,
safeguarding children's lead, matron, ward sister/change nurse, site manager, consultant.

2. The Trust and the Department

Vascular Surgical Unit


There are currently 8 consultant surgeons, with 3.8 WTE performing vascular surgery, 4
have a Trauma commitment and one Associate Specialist.

Barts Health NHS Trust is committed to a model of surgical management which involves
clinicians. The vascular surgical department is part of the Surgical Division. The department
and division work closely with the School of Medicine and Dentistry and the William Harvey
Institute to promote clinical translational research. We also have well established links with
referring district general hospitals and are committed to providing high quality specialist
vascular services for the whole population of NE London and to work with the Network and
partner DGH’s to continue to improve access to vascular care.

On the Royal London Hospital site, there is a dedicated theatre for vascular surgery which
runs Monday – Friday as part of the 12 main theatres. Additionally there are day surgery
sessions for vascular surgery in the ACAD unit.

Vascular Surgery occupies a 26 bedded ward. This is located on the 10th floor of the main
Royal London Building.

Interventional Radiology and Imaging Facilities


The vascular service is supported by a strong Interventional Radiology (IR) service with
which the department has close links with. There are three IR suites and a hybrid operating
room available for complex endovascular procedures.

There is a dedicated vascular ultrasound laboratory on site and all other expected diagnostic
services (CT/MRI)

Rehabilitation
A comprehensive rehabilitation programme is provided by therapies and supported by senior
nurses.

Undergraduate Education
The Vascular surgical department is responsible for the education of undergraduate medical
students from Bart’s and The London School of Medicine and Dentistry. We also accept
students from other parts of the UK and elsewhere in the World for elective periods.

Post Graduate Education


Education and training for our medical staff is given high priority at Barts Health NHS Trust.
The post-graduate meetings, courses and workshops are open to all the hospital junior staff
who are encouraged to attend and participate in the programme.

Audit
Audit is well established and takes place once a month (in association with the regular
teaching day) and looks at specific clinical activities. The meetings are chaired by the
Clinical Director and the Audit/ Governance lead. Presentations are given by junior staff in
rotation.
