    This is to Authorize my brother Andree Mar B. Distor to claim my Form 137 and Diploma on my
    behalf. I will not be able to get my documents since I am in Manila. I would need an original
    copy of my form 137 and Diploma to process my employment to work on Abroad.

    Attached is a copy of my identification for your reference . Thank you and I am hoping for your
    smooth cooperation.
