    PETITION

    COMES NOW the petitioner and before this Reconstituting Officer


    thru the Register of Deeds, respectfully states:

    1. That petitioner is of legal age, married, and resident of Atlag,


    Malolos City, Bulacan;

    2. That the petitioner is one of the heirs of the deceased registered


    owners of a parcel of land registered in the name of Jose Villanueva married
    to Magdalena Jose, covered by and described in Transfer Certificate of Title
    No. T-139044 of the Registry of Deeds of the Province of Bulacan. A copy
    of the Special Power of Attorney issued in favor of petitioner by all the heirs
    of the registered owners is hereto attached as Annex “A”;

    3. That the original of the aforesaid Original Transfer Certificate of


    Title on file with the Registry of Deeds of Bulacan Province was lost or
    destroyed due to the fire which happened on ________________;

    4. That submitted herewith are the following papers in support of this


    petition:

    (a) Owner’s Original Duplicate Transfer Certificate of Title and


    three (3) photocopies thereof (Annex “B” and “B-1”;
    (b) Real estate tax receipt representing full payment of at least
    two (2) years prior to the filing of the petition; (Annex “C”
    and “C-1”)

    (c) Tax Declaration (Annex “D”);

    PRAYER

    WHEREFORE, it is respectfully prayed that the original of Transfer


    Certificate of Title No. 139044 lost or destroyed in the files of the said
    Registry of Deeds be ordered reconstituted on the basis of the owner’s
    duplicate transfer certificate of title and in accordance with Republic Act
    No. 6732.

    Francisco J. Villanueva after having been duly sworn to in accordance


    with law, deposes and says: that he is the petitioner; that he caused the
    preparation of the foregoing petition; that he has read the contents thereof
    and the facts stated therein are true and correct to his knowledge.
