Checklist for Blood Bank

medicine or hematology shall manage the
blood collection/distribution department.
2 Donors are appropriately screened prior to
blood donation.
3 A couch or cot needs to be provided for
the donor for the donor and other

in the event of a donor reaction.
5 Refrigerators, warmers and freezers must
have temperature monitoring devices with
alarm systems and constant UPS backup
power.

blood and blood products.

8 Policies and procedures for blood bank are
present and known to all staff
9 Appropriate disposal of blood and blood
products.

collection/distribution register/ distribution
tracking system exists.
11 There is an evidence of hospital
transfusion committee with defined terms
of reference, reporting lines and minutes of
meetings.
12 Donated blood is labeled appropriately
with adhesive labels.

14 Whole blood and red blood cells are stored
between 2 and 6 degree Centigrade.
15 Fresh frozen plasma is stored at -25
degree centigrade.
16 Standard pre- testing of recipients
including emergency patients are applied
at all times with documented audits of all
blood transfusions. Pre testing record
completed for each recipient consisting of
ABO typing, Rh (D) group, Anti body
screen & Cross Match.
17 Register of all recipient adverse reactions
to blood and blood products.
18 Data collected regarding recipient adverse
reactions is collated, analyzed and
reported to the blood transfusion
committee.
19 All steps of the administration of blood and
blood products to be performed by two
members of qualified nursing staff and
documented.
20 An approved blood warming device is
used and blood warming device are
monitored so that blood is not warmed
above 38 degree centigrade.
21 Staff ratios must be sufficient
