    AFFIDAVIT

    I, RICAREDO DATINGUINOO DALISAY, Filipino, of legal age, married, PNP


    member presently assigned at San Pascual Municipal Police Station San Pascual,
    Batangas and resident of Taysan, Batangas after having been sworn in accordance with
    law, hereby depose and state that:

    1. I was designated as Officer-In-Charge of San Pascual, Municipal Police Station


    since September 2022 to date.

    2. That on December 2, 2022, the undersigned received a radio message from your
    office directed to explain in writing why it appeared in CIRAS upon verification that
    one (1) case on Illegal Gambling operation in San Pascual, Batangas was returned
    for further investigation (RFI) for lack of interest of the complainant.

    3. As Chief of Police, I always reminded the Investigation Section headed by PCMS


    Francisco Gabarda Bonado Jr, to monitor and supervised encoding of incidents at
    Crime Incident Reporting and Analysis System (CIRAS). He also reminded CIRAS
    Encoders to update all cases after receiving orders or resolution from Court.

    4. That, a case of Violation of RA 9287 (Bookies) was filed against Charlie Liboon y
    Roxas on September 13, 2022 with docket number IV-02-INQ-22I-01632 before
    Prosecutor Jocelyn E. Perez-Villagomez at RTC, Batangas. That this case was
    DISMISSED DUE TO LACK OF PROBABLE CAUSE on September 13, 2022.

    5. That, by religiously adhering to the mandate and command on updating of cases, it


    appears while updating the case of illegal gambling unintentionally while choosing
    from the drop down options had clicked on the lack of interest of complainant. It was
    without any intention to provide false/inaccurate report but purely human error.

    6. That Chief, Investigation is reminded to supervised and monitor CIRAS Encoders to


    double check entries before saving and provide accurate data at CIRAS.

    7. Furthermore, the undersigned will make sure that this will not happen again in the
    future.

    8. The undersigned affiant is executing this affidavit to attest to the truth of the
    foregoing, and for whatever legal purpose that it may serve.

    IN WITNESS WHEREOF, I hereby affix my signature on this 2nd the day of


    December 2022, in San Pascual Municipal Police Station, San Pascual, Batangas,
    Philippines.
