    JOINT AFFIDAVIT OF TWO DISINTERESTED PERSONS

    WE, MARIO AGNO VILLAR and MARLON ROXAS VERTUDEZ, both of


    legal ages, Filipino Citizens, and residents of Brgy. Anawan, Polillo, Quezon, after
    having been duly sworn to in accordance to with law, depose and says:

    That we personally know RENE ILUSTRISIMO MOLIHON of Brgy. Casay, San


    Francisco, Quezon, being an acquaintance for a long period of time and the fact that he is
    the biological father of RICHELLE G. MOLIHON;

    That in the Certificate of Live Birth of RICHELLE G. MOLEHON, his (RENE


    ILUSTRISIMO MOLIHON) Surname was erroneously entered thereon as MOLEHON
    instead of his true and correct SURNAME which is MOLIHON;

    That in all his Certificate of Marriage, scholastic records, business transactions and
    other dealings, he had been using MOLIHON as his Surname;

    That RENE ILUSTRISIMO MOLIHON and RENE ILUSTRISIMO MOLEHON


    refers to one and the same person;

    That We are not related by affinity or consanguinity to RENE ILUSTRISIMO


    MOLIHON; and

    That we executed this Affidavit to attest to the truth of the foregoing facts and
    circumstances and to inform the proper authorities that RENE ILUSTRISIMO
    MOLIHON and RENE ILUSTRISIMO MOLEHON refers to one and the same person.

    IN WITNESS WHEREOF, we have hereunto set our hands and affixed our
    signatures this 29th day of July 2023 at Lucena City, Philippines.
