    TWO DISINTERESTED PERSON

    We, _________________, of legal age, Filipino citizen, married, and resident LPV, Bogo
    City, Cebu, Philippines and S___________________, likewise of legal age, Filipino citizen,
    widow and resident of Gairan, Bogo City, Cebu, Philippines, after being duly sworn to in
    accordance with law, hereby depose and state:

    1. That we have personally known the late LEONARDA DAMAYO SICLOT, being our
    neighbor and friend for a long period of time;

    2. That we know for the fact that the late LEONARDA DAMAYO SICLOT died on July 2,
    1997 at Maranagog, Bogo City, Cebu, Philippines;

    3. That we hereby declare, confirm and affirm that LEONARDA DAMAYO SICLOT died
    on July 2, 1997 at Maranagog, Bogo City, Cebu, Philippines;

    4. That we are not related to them by blood or by affinity nor we received any monetary
    consideration from their family;

    5. As such, we are executing this Affidavit to attest to the truth of the foregoing.
