This correspondence is to officially inform you that you are hereby suspended from your
job without pay for three (3) working days. Your suspension will begin on March 20, 2023
and will end on March 22, 2023. You are being suspended for violation of Code of Employees
Discipline Article VI Section 3.1.3 “tardiness for three (3) times within a month”.

You have received previous reprimand for violation of incurring more than three times of
tardiness also for the month of July 2022. Despite repeated reminders to improve your
attendance and punctuality, you have repeated the violation as follows:

Office Memo No. 044 s. 2023 dated January 31, 2023 was issued notifying you to explain
within five (5) working days upon receipt in reference to the above stated violation. A copy of
response to the memo was received by the undersigned last February 15, 2023.

Based on the explanation, we regret to inform you that your tardiness for November 2, 8
and 14, 2022 is not justifiable nor acceptable considering that there are numerous employees
coming from the same or even farther areas but had managed to arrive earlier than the required
time of duty.

Your conduct described above constitutes sufficient cause for the disciplinary action of
suspension without pay even though the provision for this violation carries only a penalty of
deduction equivalent to one (1) day and a reprimand. As per record, you have been
reprimanded for your first violation as per Office Memo No. 336, s. 2022 dated September 30,
2022. This is already considered second offense for the same violation and under the rule, it

merits a stiffer penalty than what is stipulated pursuant to Article VIII Section 2 of COED “the
penalty for successive violations of the same rule is progressively more severe than the
penalty for first violation”.

The purpose of this suspension is to impose discipline with regards to the above
violation of our policy and to give you the opportunity to reflect upon your future compliance
with our employment standards. One more violation of the conditions of your employment will
result in a more serious sanction including termination. A copy of this order will be placed in
your 201 file.
