    PETITION

    PETITIONER, through the undersigned counsel, unto this Honorable


    Court, most respectfully alleges – THAT:

    1. Petitioner, ROWENA MANCERA MARAMO, Filipino, of legal age,


    married, and a resident of Barangay Bagongpook, Lipa City Batangas, who
    can be served with the Court’s notices and other legal processes through
    the undersigned Counsel at 255-A Macasaet Street, Bagongpook, Lipa
    City, Batangas;

    2. The OFFICE OF THE CIVIL REGISTRAR OF LIPA CITY, BATANGAS


    is being impleaded as required by law, which holds office at the vicinity of
    the City Hall of Lipa, with an address of Main Building, Maraouy
    Highway, City Hall Compound, Lipa City, Batangas, where it may be
    likewise served with summons and other legal processes of this Honorable
    Court respondents Other Persons are parties in interest who may be
    affected by this instant petition and herein impleaded in compliance to
    Section 3, Rule 108 of the Rules of Court;

    3. Petitioner is one of the children of Spouses _________ and ________ who


    were married on ___________.

    4. Petitioner was born on “August 27, 1969” at their residence house in Brgy.
    Bagongpook, Lipa City, Batangas, through the help of a local
    “manghihilot”.

    5. Petitioner’s fact of birth was belatedly registered on May 16, 1970 but
    erroneously entered “April 5, 1970” as petitioner’s date of birth, before the
    Office of the Civil Registrar’s Office of Lipa City, Batangas under Civil
    Registrar-General No. 1106212. A duly authenticated copy of the
    Certificate of Live Birth of ROWENA MARAMO MANCERA is hereto
    attached as Annex “A” and made an integral part hereof;

    6. Petitioner had been using her correct date of birth ever since in all her
    records including petitioner’s Government ID’s such as Philippine Health
    Insurance Corporation (PhilHealth) ID, Professional Regulation
    Commission (PRC) ID, Unified Multi-Purpose ID, Certificate of Marriage,
    MEC Form 137, and Report of Rating for National Board for Teachers a
    copy thereof are hereto attached as Annexes “B”, “ C”, “D” and “E” and
    made an integral part hereof;

    7. A Judicial Affidavit of __________________ and ____________ was issued


    by in support to this petition. A copy of the Judicial Affidavit is hereto
    attached as Annex “F” and “G” and made an integral parts hereof;

    8. In order to correct the aforementioned misinformation and erroneous


    entry in her Certificate of Live Birth, Petitioner files the instant Petition
    pursuant to Rule 108 of the Revised Rules of Court;

    PRAYER

    WHEREFORE, premises considered, it is most respectfully prayed to this


    Honorable Court that after due notice and hearing, an Order be issued directing
    the herein Respondent Office of the Civil Registrar of Lipa City to correct the
    information supplied in item “6. Date of Birth” from “April 5, 1970” to “August
    27, 1969” in the Certificate of Live Birth under Registry No. 1106212 of Petitioner.

    Other reliefs consistent in law and equity are also sought.

    I, ROWENA MARAMO MANCERA, of legal age, Filipino citizen, married


    and a resident of Brgy. Bagongpook, Lipa City, Batangas, after having been duly
    sworn to in accordance with law do hereby depose and say that:

    1. I am the petitioner in the above-entitled case; that I have caused the


    preparation of the foregoing petition and have read the allegations
    contained therein; that the allegations in the said petition are true and
    correct of my own knowledge and authentic records in my possession; I
    hereby certify that I have not commenced any other action or proceeding
    involving the same issues in any court, tribunal or quasi-judicial agency
    and, to the best of my knowledge, no such other action or claim is pending
    therein;

    2. If I should learn thereafter that a similar action or proceeding has been


    filed or is pending, I hereby undertake to report that fact within five (5)
    days therefrom to the court or agency where the original pleading and
    sworn certification contemplated herein have been filed; that I executed
    this verification/certification to attest to the truth of the foregoing facts and
    to comply with the provisions of Adm. Circular No. 04-94 of the
    Honorable Supreme Court.
