    UNDERTAKING

    Pursuant to Section 17 in relations to Section 2 of Rule 114 of the 1995 Rule on


    Criminal Procedure, I hereby agree to the following conditions;

    A. That the undertaking shall be effective upon approval and remain in force at all
    stages of the cases until its final determination unless the proper Court directs
    otherwise;

    B. That I shall appear before the proper Court whenever so required by the Court;

    C. That failure on my part to appear at the trial without justification despite due
    notice shall be an express waiver of my right to be present on the date specified in
    the notice. In such case, the trial may proceed in absentia;

    D. That I shall surrender myself for execution of the final judgment.

    E. That I shall not leave the country without prior permission to this Honorable
    Court, otherwise my passport may be ordered cancelled; and

    F. That I further undertake to notify the Court of any change of my esidence,


    otherwise, Paragraph C above shall apply.
