SUMMATIVE FORM

Instructions:
A. For the Training Officer / Training Core Faculty:
a. Prepare a simulated case scenario together with laboratory and procedure results for interpretation by the
residents, and guide questions and expected responses (examiner’s copy).
b. The case must be appropriate for the level competency of the examinees.
c. Discuss the case with the examiners before the start of the oral exam. Inconsistencies about the case and
responses must be resolved before the actual examination.
d. Determine the appropriate duration of the oral exam for each case.
e. Assign paired or triple examiners for each examinee.
f. The examinees must be briefed on the mechanics of the oral exam including the criteria for scoring.
g. The minimum pass level (MPL) is 7.
h. Examinees with unsatisfactory performance must undergo another oral exam until he/she gets a score of 7
or above.
B. For the examiners:
a. Provide enough time for the resident to read and study the case before starting to ask questions.
b. Ask questions in chronological orders. You may ask other relevant questions to prove further into the
responses of the examinee.
c. Write the responses of the resident in the space provided for. Check whether you agree or disagree with the
responses of the resident. Write very brief comments or feedback for each dimension being discussed.
d. Encircle your score for each dimension.
e. Avoid making unnecessary comments during the examination.
f. Compare your scores with the other examiner/s. If scores for a particular dimension have a wide gap,
discuss it but scores must still be individualized.
g. Add individual scores and divided by 10. Get the average scores of all the examiners to determine the final
score of the examinee for a particular case.
h. Give immediate feedback to the examinee.
