PETITION
PETITIONER, Jam T. Raquion, unto this Honorable Court, most respectfully

states that:
1. Petitioner is 27 years of age, a Filipino citizen, single, and a resident of #31

Howard St. Brgy. 13 San Eugenio, San Nicolas, Ilocos Norte. Petitioner’s
date of birth is January 14, 1995. Copy of her Certificate of Live Birth is
herewith attached and marked as ANNEX “A” and made an integral part of
this petition;
2. Petitioner maintains a regular place of work at Brgy. 15 San Jose San

Nicolas, Ilocos Norte, along the National Highway, in front of Franky Car
Aircon Service Center, where she may be served with summons and other
processes of this Honorable Court. Petitioner may be reached through her

3. Petitioner has satisfactorily completed the required course in law at

Northwestern University Laoag City, College of Law in 2020 and has passed
the 2020-2021 Bar Examinations and admitted to the practice of law in the
Philippines on May 21, 2022. Copy of the Certificate of Bar Membership is
herewith attached and marked ANNEX “B” and made an integral part of this
petition;
4. Petitioner is a member of the Philippine Bar in good standing with

Clearances from the Office of the Bar Confidant of the Supreme Court
and the Integrated Bar of the Philippines (National). Copies of the
clearances thereof are herewith attached and marked as ANNEX “C” and
ANNEX “D”, respectively;
5. Petitioner has not been charged nor convicted in the first instance of any
crime involving Moral Turpitude, nor charged in any administrative case
before any court or tribunal in the Philippines as evidenced by her
Clearances issued by the Municipal Trial Court of San Nicolas, the
Regional Trial Court and National Bureau of Investigation (NBI).
Aforementioned clearances are herewith attached and marked as ANNEXES
“E”, “F” and “G”, respectively;
6. Petitioner has also obtained a Certification of her good moral character

from two (2) executive officers of the Integrated Bar of the Philippines
(IBP) – Ilocos Norte Chapter where she is a member. Said certification is
herewith attached and marked as ANNEX “H”;
7. Petitioner has paid the required membership dues for calendar years 2022 and
2023 as evidenced by a scanned copy of IBP Official Receipt Nos. 217147
and 217148 both issued on May 25, 2022 herewith attached as ANNEX “I”
and ANNEX “I-1”;
8. Petitioner has likewise paid her Professional Tax for the year 2022 under
Official Receipt No. 1307170 dated June 14, 2022 at San Nicolas, Ilocos
Norte. A photocopy of the Receipt is herewith attached and marked as
ANNEX “J”;
9. Petitioner has likewise secured her Community Tax Certificate No.

07279326. A photocopy thereof is herewith attached and marked as ANNEX
“K”;
10. Petitioner herewith attached three (3) specimens of her signature and
three (3) samples of the true impression of her dry seal marked as
ANNEXES “L” and “M”, respectively;
11. Petitioner likewise attached herewith three (3) passport-size photographs

taken within thirty (30) days from the date of this application and with
Petitioner’s signature at the bottom of each photograph marked as
ANNEX “N”;
12. Petitioner has not applied nor have been commissioned as Notary Public in
any other jurisdiction;
13. Petitioner possesses all the qualifications and none of the disqualifications
for said office and have read and understood, and willing to abide by the
2004 Rules on Notarial Practice;
14. Petitioner prays to be commissioned as Notary Public for and in the

Municipality of San Nicolas and in the Province of Ilocos Norte,
Philippines;
PRAYER
WHEREFORE, premises considered, it is most respectfully prayed unto

this Honorable Court, after due notice, publication and hearing, THAT:
1. The Petition for Commission as Notary Public filed by the undersigned

be acted favorably and GRANTED; and
2. Petitioner be commissioned as a Notary Public for and in the

Municipality of San Nicolas and in the Province of Ilocos Norte,
Philippines.

preparation of foregoing Petition; I have read and understood the
contents thereof; The allegations in the pleading are true and correct
based on my personal knowledge and on authentic documents; The
pleading is not filed to harass, cause unnecessary delay, or needlessly
increase the cost of litigation; And the factual allegations therein have
evidentiary support or, if specifically so identified, will likewise have
evidentiary support after a reasonable opportunity for discovery;
2. I have not filed any similar petition before the Supreme Court, the
Court of Appeals, or any division thereof or before any court or
quasi-judicial tribunal involving the same issues; I know of no
pending similar petition before said SC, the CA, or any division
thereof, or in any court or quasi-judicial body; In case I shall come to
knowledge of any such similar petition filed and pending before the
SC, the CA, or any division thereof, or any court or quasi-judicial
tribunal, we shall inform this Honorable Court within five (5) days
from the same.
