    I, ROLANDO V. VILLANUEVA, of legal age, Filipino Citizen and a resident of No. 19


    Kisad Road, Baguio City, after having been duly sworn to in accordance with law, do hereby
    depose and say that:

    1. I am the sole proprietor of the RVV Shell Stations located at #1 Naguilian Road,
    Kayang Extension, Baguio City, #75 Duay Street, Abanao Road, Baguio City, Km. 4 Marcos
    Highway, Bakakeng Central, Baguio City, and at FA 287, Km.4 Balili, La Trinidad, Benguet;

    2. I am changing the name of my business from XXX to XXX;

    3. I am executing this Affidavit to attest to the truth of all the foregoing statements,
    and for any and all legal intents and purposes it may serve.
