Introductions

Practicums/Internships are the bridge between academia and the professional world. They

provide students with an opportunity to gain professional skills that will enhance marketability upon

graduation from college. The word internship refers to a short-term work experience when a student

participates in a program of temporary, supervised work in a particular field in order to gain practical

experience. It incorporates education and professional development and is designed to guide the intern

toward a professional career in a particular organization, field, or sector. The word practicum refers to a

course involving activities emphasizing the practical application of theory, especially one in which a student

gains on-the-job experience in a field of study

Nurses is a healthcare profession that focuses on the care of individuals and their families to help them

recover from illness and maintain optimal health and quality of life. Nurses are distinct from other

healthcare providers as they have a wide scope of practice and approach to medical care. They play an

integral role in promoting health, preventing illness, and caring for all individuals, including those who are

disabled or are physically or mentally ill. The majority of nurses work in a hospital environment and provide

medical care to patients with illnesses in this environment. However, there are various other possible

workplaces of a nurse, including residential care homes, in-home care services, governmental services,

military, schools, and research facilit 3.

As student in Master of Art in Nursing Program in Wesleyan University –Philippines I am

Chito Wayne A. Esponilla,RN will be conducting my practicum in Northern Samar Provincial Hospital in

Operating Room Department which I am currently working as Senior Operating Room Nurse under the

supervision of my site supervisor Ms. Flor M. Cebujano, RN after this practicum, I will be able to learn new

insight in leadership , and management style , and enhances my capability to handle situations like making

staff schedule, surgical case reporting, surgery census and requesting surgical supplies and equipments.

As a profession, nursing has several ethical principles that guide their philosophy of practice

throughout all tasks in the workplace. These are to Provide exemplary care at all times Prioritize the care of

patients with respect to individual dignity Cooperate with other health professionals to improve patient

outcomes Act with integrity and honesty to maintain the profession reputation

In accordance with these principles, nurses are able to care for patients in the best way possible to

promote positive health outcomes. The physical demands of the workplace are significant, and nurses are

prone to back injuries due to the need to lift and move objects or patients around the workplace.

Additionally, nurses are at risk of infection or illness due to exposure to bacteria, viruses, and hazardous

drugs or substances in the work environment. To minimize this risk, appropriate sanitization and safety

guidelines should be followed.

Given that continuous care is required for patients in hospital, nurses usually work in shifts to

provide round-the-clock care. Although the work schedule differs according to individual workplace, nurses

are usually required to work night shifts and on holidays or weekends. This can also disrupt sleeping

patterns and, in some cases, lead to shift-worker sleeping disorder

Province of Northern Samar

Northern Samar Province is one of the three provinces comprising Samar Island. It is bounded by the

Pacific ocean on the east; the San Bernardino Strait on the north; Samar Sea on the west; and the Samar and

Eastern Samar provinces on the south. It is 733 kilometers away from Manila by land. Its total landmass is

349,800 hectares, of which about 52 percent is covered by forest and 42 percent is classified as alienable and

disposable. The terrain is very rugged with low-lying river valleys and hills crossing one another. The

province falls under the intermediate type climate, which has no distinct dry and wet seasons.

It is divided into two legislative districts, comprising 24 municipalities and 570 barangays, with its

capital named Catarman which is also the political and economic center of the province. The main languages

are Norte-Samarnon, Cebuano and other local dialects.

Its major industries are agriculture, fishery and handicraft production. The agriculture includes rice and

vegetable farming, cattle raising, banana and peanut plantation and oil manufacturing, The fishery consists

of deep sea fishing, prawn and agar culture and fresh water fish culture,. The handicraft production

comprises furniture manufacturing, hat and mat weaving, brick making and soap processing

Northern Samar covers a land area of several square kilometers from the Pacific Ocean in the East to

the Samar Sea in the West. The “Balicuatro Islands” of Biri, Capul, San The Coat of Arms of Samar

represents the political, geographical description, historical, economic, and social representation and

allegorical ideas of the province. The letters N and S represent Northern Samar. The map of Northern

Samar represents the geography and 24 municipalities of the province, including the five island towns.

The galleon represents the Spanish conquistadors that reached the land of the Ibabao to preach the gospel of

Christianity, spread the Creed of Roman Catholicism and introduce civil governance thru the Royal Port of

Palapag in 1640. Mount Bubuya (Palapag Mesa) represents the highest mountain range in Palapag,

where Agustin Sumuroy and his men retreated to and encamped after killing Fr. Miguel Balberan, thus

starting the Sumuroy Rebellion. The rice field, abaca, timber, and coconut are all economic representations.

The following are the allegorical ideas of the province: Blue, the color is symbolic of vast marine and

aquatic resources, a source of livelihood for the fisher folks of the coastal towns. Tangerine represents the

cheerfulness, high spirits, and optimism of the people of the province. Yellow represents golden harvest, and

abundance of resources. White symbolizes transparency in governance. The torch is meant to illuminate the

province and set it afire with quality education, a primary thrust of the provincial government. Rope ties the

emblems together in a circular shape, it represents equality in rights and justice, and unity for peace and

development.

Antonio & San Vicente offer an amalgam of adventure and nature – each with their own unique tourism

jewels.Biri’s attraction are the seven geologic Rock Formations that are breathtaking similar to Australia’s

12 Apostles. Capul, on the other hand, is replete in history as an anchorage for the Manila-Acapulco Galleon

Trade in the 17th century. A Fortress Church is still relatively intact in the town proper and on the northern-

most tip stands the Faro standing like a rampart to the Golden East. One can also learn Capul’s unique

dialect known as “Abaknon” a derivative from the Austronesian language of the Sama-Badjao.San Vicente

prides

itself with its Pink beach beach in Sila Island. San Antonio is the most developed of the four island-

municipalities with a chain of resorts spanning the entire eastern shores of the island. Northern Samar’s

capital, Catarman, is home to the University of Eastern Philippines, a repository of the region’s rich

history.In Palapag one can see the remnants of a town still steeped with its historicity tied to the Sumuroy

rebellion. The Church ruins are a sight to behold coupled with the centuries-old Watch Tower. In 1999, the

internationally - renowned ELF AUTHENTIC ADVENTURE started its run in Northern Samar’s

hinterlands due to the varied challenges it offered.

Northern Samar is one of the three Samar provinces created on June 19, 1965 by virtue of Republic

Act No. 4221. The province is relatively young but it has vital religious and historical significance. The

small island of Capul was the capital of the province of Samar in 1848-1852. Capul was formerly named

Abak after the ancient ruler of Java who brought the first settlers to the enchanting island. In the latter period

of the 16th century, it was among the first places to be evangelized by the Spaniards (1596). The waray-

warays of Northern Samar also figured prominentlt during the Spanish and American occupation of the

Philippines. It was part of the route of the galleon trade and the Sumoroy Rebellion started in palapag (1649-

1650).

The festival will be launched on June 4 with a grand float parade participated in by the 24 component

municipalities around the capital town Catarman.Northern Samar, referred to in the olden days as Ibábao,

was known for the ancient valor and bravery of its people.

the place was thereafter formally known

under its new name until today.He added

that the provincial government is

Norther

n Samar covers a total area of 3,692.93 square kilometres (1,425.85 sq miles)occupying the northern section

of Samar Island in the Eastern Visayas region. The province is bounded by north by the San Bernardino

Strait, on the east by the Pacific Ocean, on the west by the Samar Sea, on the southwest by Samar and on the

southeast by Eastern Samar. It ranks thirty-seventh (37th) in size among the 80 provinces of the Philippines

and accounts for practically 1.2 percent of the total land area of the country. About 52 percent of the total

land area is covered by forest and 42 percent is classified as alienable and disposable.

The province is composed largely of low and extremely rugged hills and small lowland areas. It also has

small and discontinuous areas along the coasts and its rivers are usually accompanied by alluvial plains and

valleys. The province is endowed with relatively rich and fertile soil that most crops can grow on

Good day, I'm Chito Wayne A. Esponilla, a PeriOperative Registered Nurse. from Purok 6 Brgy,

age 32. Dalakit Catarman Northern Samar I attended Dalakit Elementary School and Catarman National

High School, where I graduated with honors. I then decided to pursue a bachelor's degree in nursing at

Southwestern University in Cebu City, where I currently working at Northern Samar Provincial Hospital

currently assign at the Operating Room Department. I chose this career path because performing surgical

work in the nursing field increases one's sense of fulfillment.

MY OBJECTIVE:

To provide competent nursing care which is safe, of highest quality, in accordance with the agency policy

and standard nursing care.

JOB FUNCTION:

Responsible for the planning, execution, directing and evaluating direct and indirect nursing care given to

the surgical patient. This nursing care ranges from preoperative intervention on to postoperative evaluation.

It is a practice design to meet a patient’s needs through the nursing process, ensuring continuity of care

during the surgical experience. 10

JOB DUTIES AND RESPONSIBILITIES

 Supervises and directs patient care in an individual operating room


 Ensures that proper techniques are used according to accepted standards of practice
 Supervise other personnel in the room and directs or assist as necessary
 Immediate reports any unusual occurrences to charge personnel, documents appropriately in the
patient records, and completes Hospital Incident Reports form if indicated
 Performs scrub duties as necessary and to keep current
 Maintains count on sponges, needles, and instrument to ensure no loss of items according to present
policy
 Performs all circulating duties with efficiency and speed
 Supervises and assists scrub nurse as necessary
 Positions patient. Prepares operative area, and adjust table to proper position according to standard
policies
 Has all supplies and equipment in room and provides to team as necessary
 Opens all sterile packs and supplies according to aseptic technique

 Assist surgeons, anaesthesiologists and other personnel as needed to ensure good patient care

On this Day , I was with my supervisor discussing about the agenda and the topic was conducted

during our convention in manila in held in the Manila Hotel and conducted by the (ORNAP) or the

and staffing about exodus of trained staff , we discuss about on how we will be ready and prepare in case of

short staffing in our area which is very difficult to handle because we are using team nursing.

My supervisor reminds me that my generation is capable and adaptable to the current technologies,

whereas their generation is having difficulties handling this but they have best practices and traditional ways

that can be useful now a days. She said that every person, regardless of the generations gaps, should address

the current trends in operating rooms regarding the staff's generation gaps in handling the most advanced

machine that we are using at the area.

Our Supervisor discussed to me about the rampant abensteesiem an tardiness of the staff she

emphasize that this should not be tolerated in can cause delays’ of the surgeries and malfunction of the

team in area she said that all off this staff is subject for the disciplinary action , everyone must be punctual

in duties and must think their job responsibilities and should be compassionate to their profession.

Replacement of Operating Room Supplies

During the morning shift endorsement my supervisor assign me to request and replenish all the supplies

needed , so I do a list of list of OR supplies and have a request to the Central Supplies Office but before I

do and ocular inspection around the OR supply room a while waiting for CSO I arrange and organize the

cubicle after that I make and inventory list to expiry supplies and and need to discard , when the supplies

arrive I receive and counter sign the recieble papers and supplies and after that i place the supplies to the

respected cubicle and and steel cabinet and I informed my supervisor .

Making Operating Room Staff Schedule

My supervisor gave me instructions to create a schedule for the shifting. The first thing I did was to

compile a list of the staff members' requests for leave of absence, special leaves, and requested shifts. I then

created a draft schedule with an equal distribution of senior nurses, team leaders, and staff members, and

submitted it to my supervisor for review and approval. She then put her signature on the schedule.

Organizing the Operating Room Set Up

The big surgical case is performed in Operating Room Number Two, thus my shift's first hour was

spent there with my supervisor. She lectures on how the operating room set up to be organize base on

surgical standards , OR tables , Suction Machine , Anesthesia Machine etc . she emphasis the importance

Operating Room- The operating room (OR) is a sterile, organized environment. As a health care provider,

you may be required to enter the OR during a surgical procedure or to set up before a surgical procedure. It
