    AFFIDAVIT

    KNOW ALL MEN BY THESE PRESENTS:

    We, SPS. _______________________, both of legal age, Filipino citizens with residence and postal
    address at _________________, after having been duly sworn to in accordance with law, hereby depose and
    state that:

    1. We have an approved loan (the “Loan”) with BDO Unibank, Inc. (BDO), which Loan will be
    secured by a mortgage over a property located at _____________________________;
    covered by and more particularly described in Transfer Certificate of Title No. 075-
    2015002092 of the Registry of Deeds of San Pablo City (the “Property”);

    2. The registered owner of the subject property appearing on aforesaid title is “ ______________,
    MARRIED TO _________, OF LEGAL AGE, FILIPINO CITIZEN whereas it should be _______,
    MARRIED TO _____________, BOTH OF LEGAL AGE, FILIPINOS.”

    3. We hereby undertake to indemnify and forever hold BDO, its directors, officers, and employees, free
    and harmless from any and all actions, losses, damages, claims, liabilities of whatever kind and
    nature to BDO, any third person or entity which may arise in connection with or as a consequence of
    the failure to indicate in the title the correct personal circumstance/s of the registered owner/s, our
    execution of a real estate mortgage on the property covered by TCT No. 075-2015002092 in favor of
    BDO, and BDO’s grant of the Loan and the additional loan/credit facility, despite the failure to indicate
    our personal circumstance/s in the title;

    4. That we are executing this Affidavit to attest to the truth of the foregoing facts and for purposes of
    correcting the title to the Property to indicate the personal circumstance/s of the registered owner.

    FURTHER AFFIANT SAYETH NAUGHT.
