    in accordance with law hereby depose and state:

    That I hereby hold myself liable and accountable should there be any
    party aside from me claiming ownership of the above-mentioned tricycle;

    That in consideration of the forgoing I am requesting from the Land


    Transportation Office that the above-mentioned vehicle be registered as
    motorcycle;

    That I execute this affidavit to affirm the truthfulness of the foregoing facts
    and for whatever legal purpose it may serve.
