Post Summary:

This post arises from a current vacancy and provides the applicant with the opportunity to
work with a lovely team providing person-centred inpatient care to older people from East
Lothian, based in East Lothian Community Hospital. This post would particularly suit
someone interested in pursuing a career in Medicine, Medicine of the Elderly or General
Practice.

The successful candidate will join the current multidisciplinary Medicine of the Elderly Team
who support the inpatient acute assessment, rehabilitation and Health Board Complex
Clinical Care (HBCCC) wards. The new post holder will provide medical input to the
orthogeriatric rehabilitation ward at East Lothian Community Hospital, supporting the existing
senior medical staff and providing medical support. This ward, (Ward 5) provides Pan-
Lothian rehabilitation to patients in the early post-op period after orthopaedic trauma.
Patients are transferred from the acute site from Day 3 onwards and so you may be
managing some post-op complications such as pneumonia, wound infections and anaemia.
The successful candidate will also provide cross-cover of other general MOE areas. There
is a very strong ethos of multidisciplinary team working within the hospital so it is important
that any candidate for this role has a collaborative approach to medical practice and ideally
experience of working within a MDT with physiotherapy, occupational therapy, nursing
colleagues and other Allied Health Professionals.

There will be the opportunity to take part in teaching, quality improvement work and service
development.

Relevant experience in care of the elderly and/or general medicine is desirable. The post will
involve some weekend commitments in Medicine of the Elderly at the Royal Infirmary of
Edinburgh. You may also be asked to cover some evening shifts at ELCH. Evening shifts
are supported by a Nurse Practitioner with advice available by telephone from the MOE
Consultant.

The post-holder will be on a standard 10 Programmed Activity contract (see detailed outline
job plan in section 6).

The Medicine of the Elderly (MOE) Service within Lothian University Hospitals Division (LUHD)
provides high quality care and a comprehensive scope of services for the elderly population in Lothian
across three main hospital sites; Royal Infirmary of Edinburgh (RIE), Western General Hospital
(WGH) and St John’s Hospital (SJH). These comprise of; acute inpatient assessment, acute stroke
services, general rehabilitation and specialist rehabilitation (stroke and orthopaedic), liaison work to
other departments and a range of outpatient services including day hospital, geriatric clinics and
specialist Parkinson services. Additional inpatient and outpatient MOE services are also provided to
Lothian patients by Edinburgh, East and Midlothian Health and Social Care Partnerships.
Additionally, there are a number of Health Board Complex Continuing Care Units (HBCCC) for
patient’s whose long-term needs cannot be met outwith a hospital setting.

MOE provides a comprehensive service to older people within Lothian, concentrating on those who
have complex needs and require specialist input. There is a clear nationally driven goal to provide
safe, high quality care to the ageing population enabling older people to remain at home for as long
as possible and limiting hospital stays to acute phases of illness. The goal of the MOE service in
Lothian is to support patients to maximise their functional potential or recover to an optimal level of
health with the view of maintaining an independent life at home or alternatively in a suitable home in
the community. This is achieved by way of multidisciplinary team working: Medical staff working
alongside nursing staff, allied health professionals and Health and Social care colleagues to provide
comprehensive geriatric assessment (CGA) There is no specific age limit on the patients seen,
though the majority seen are in the age range 80 plus.

Within East Lothian there is a strong ethos of community working and trying to manage patients within
East Lothian where possible. The East Lothian Community Hospital, 37 miles from Edinburgh city
centre, provides ongoing MOE rehabilitation for East Lothian Residents following acute admission to
the Edinburgh acute hospitals. This includes stroke and orthogeriatric rehabilitation. In addition, there
is a ward for Health Board Complex Clinical Care (ward 1). ELCH is also home to a Psychiatry of
Older Age unit which provides care for both assessment and ongoing patients. There is a small
community hospital, Belhaven Hospital, in Dunbar.

The East Lothian Hospital@Home provides hospital level care to frail or complex patients in their own
homes as an alternative to admission to the acute sector. They work closely with various community
teams including the Care Home Team, the Prevention of Admission Therapy Team, Hospital2Home
(providing care and promoting recovery after a hospital stay), District Nursing and many more.

You will have the opportunity to attend ELCH unit teaching and remotely attend MoE weekly teaching
at the Royal Infirmary of Edinburgh via Microsoft Teams.

Section 4: Main Duties and Responsibilities

The final job plan will be subject to discussion and agreement between the successful post
holder, and the current staff.

The post will involve the care of inpatients in the orthpaedic rehabilitation ward on the ELCH
site including review of new admissions and ward-based problems, supporting junior staff
and contributing to MDTMs.

We have 4th year medical students on site 2 days a week and you will be expected to
contribute to their teaching.

This post will involve some out of hours work where weekends are currently undertaken at
the Royal Infirmary of Edinburgh.

Please note this is an Exposure Prone post and satisfactory pre-employment health
screening is essential

REHABILITATION The rehabilitation of Offenders act 1974 allows people who have

However, due to the nature of work for which you are applying this
post is exempt from the provisions of Section 4 of the
Rehabilitation of Offenders Act 1974 by virtue of the Rehabilitation
of Offenders Act 1974 (Exceptions Orders 1975 and 1986).
Therefore, applicants are required to disclose information about
convictions which for other purposes are “spent” under the
provision of the act in the event of employment, failure to disclose
such convictions could result in dismissal or disciplinary action by
NHS Lothian. Any information given will be completely
confidential.

MEDICAL In terms of NHS Circular 1989 (PCS) 32 dealing with Medical


NEGLIGENCE Negligence the Health Board indemnity will cover only Health
Board responsibilities. Paragraph 63 of the General Medical
Council's Good Medical Practice requires you to have adequate
insurance or indemnity cover. You may wish to consider taking out
additional medical indemnity e.g. with a Medical Defence
Organisation to ensure that you have indemnity for the whole of
your practice.

NOTICE Employment is subject to one month notice on either side, subject


to appeal against dismissal.

PRINCIPAL BASE You may be required to work at any of NHS Lothian sites as part
OF WORK of your role.
