    AFFIDAVIT OF SELF-ADJUDICATION WITH WAIVER


    I, KAREN O. REQUINTA, of legal age, single, Filipino, and resident of
    San Juan Bautista, Goa, Camarines Sur, after having been duly sworn according to law hereby
    depose and say:
    1. That I am the sole heir being the only child of the late FLORENTINO P. REQUINTA III,
    who died intestate in Vinzons, Camarines Norte on January 5, 2017;
    2. That my father the late FLORENTINO P. REQUINTA III left an estate consisting of a
    share over a parcel of residential land with improvements thereon situated at La Purisima
    St., Goa, Camarines Sur, particularly described as follows:
    TD No. 2016-16-014-00013
    “A parcel of residential land with improvements thereon (Lot 353) situated
    at La Purisima, Goa, Camarines Sur containing an area of FIVE HUNDRED
    TWELVE (512) SQUARE METERS more or less. Bounded on the North by
    Creek; on the East by 293; on the South by La Purisima St.; and on the West
    by Pathway, and covered by Tax Declaration No. 2016-16-014-00013 for land
    and Tax Declaration No. 2016-16-014-00014 for building. That my father has
    1/6 share over the subject property.”

    3. That as far as I know, the decedent left no will, no debts or obligations that may be
    charged against his estate;

    4. That I am adjudicating unto myself the estate of my father, FLORENTINO P.


    REQUINTA III ;

    5. That I, KAREN O. REQUINTA, do hereby waive, convey and transfer by way of


    Waiver unto my Uncle, PERPETUO P. REQUINTA, his heirs and assign, my share
    and interest over subject property;

    6. That I, PERPETUO P. REQUINTA do hereby accept this waiver and do hereby express
    gratitude for the kindness and liberality of KAREN O. REQUINTA. Furthermore, I
    shall allow my niece KAREN O. REQUINTA to occupy subject property with a
    monthly rental of ONE THOUSAND EIGHT HUNDRED (₱1,800.00) PESOS in the
    next FIVE (5) YEARS and thereafter subject to TEN PERCENT (10%) increase every
    TWO (2) YEARS ;

    7. That I am executing this affidavit in compliance with the Rules of Court.
