    I, FELJAN HANNA TEJADA MADULIN, of legal age, Filipino, married, and a resident of
    Purok 14, Isabela Homes, Brgy. New Visayas, Panabo City, Davao del Norte, Philippines,
    hereby name, constitute, and appoint, FELIX LUZANO TEJADA, of legal age, Filipino, and a
    resident of Purok 14, Isabela Homes, Brgy. New Visayas, Panabo City, Philippines, to be my
    true and lawful attorney-in-fact, for me and in my name, place and stead to do and perform
    the following acts:

    1. To receive my ATM Card from BDO Network Bank- Panabo Maharlika Branch under
    Savings Account No. 040410181447 under my name;

    2. To sign any document for the claim of such ATM Card.

    HEREBY GIVING AND GRANTING unto my said attorney-in-fact full powers and authority
    to do and perform all and every act requisite or necessary to carry into effect the
    foregoing authority, as fully to all intents and purposes as I might or could lawfully do
    if personally present, with full power of substitution and revocation, and hereby
    ratifying and confirming all that my said attorney-in-fact shall lawfully do or cause to
    be done by virtue hereof.
