FAMILY DAY ACCOMPLISHMENT REPORT

SCHOOL YEAR 2022-2023
ACTIVITIES CONDUCTED:
A. The Parent-Teacher Association (PTA) held a meeting on November 18, 2022 to

discuss the upcoming Family Day event and its goal to procure funds for a school
project.
B. The PTA presented the Family Day event plan, which includes various fun and
engaging activities for families, such as games, booths (Bahay kubo), raffles, and
performances of every grade level. The event is scheduled on December 16, 2022 at
the school grounds. The PTA president emphasized that the success of the event
would greatly help in procuring funds for the school project.
C. Each Grade level conducted a meeting with the parents to discuss the Family Day.
D. The PTA officers solicited donations from businesses, organizations, and individuals
in the community in addition to the funds raised from the Family Day event.
E. The PTA officers announced the winners of the Raffle Draw, which was one of the
most anticipated activities of the event.
F. The PTA officers recognized the Bahay Kubo Winner, which was a contest that
challenged the creativity and resourcefulness of the families in building a traditional
Filipino house.
G. The PTA officers recognized the winners of each activity
H. The PTA officers recognized the effort of all parents by giving certificates.
I. The PTA officers recognized the Mass Dance Winner, which was a contest that
showcased the teamwork and coordination of the families in performing a dance
routine.
J. The PTA Meeting was also able to successfully procure the school project, which is
the sound system. After careful deliberation and canvassing, the PTA was able to find
a reliable supplier who provided high-quality equipment at a reasonable price. The
sound system was delivered and it has been used for various school activities, such
as assemblies, presentations, and performances.
K. The PTA Meeting also discussed other matters, such as the budget for the next school
year, fundraising activities, and possible projects that could benefit the school and its
students.

KING AND QUEEN OF HEARTS 2023

ACCOMPLISHMENT REPORT
ACTIVITIES CONDUCTED:
A. The PTA held a meeting on January 6, 2023 to discuss another possible source of fund
and come up with the King and Queen of Heart 2023 fundraising program. The objectives,
guidelines, and timelines of the program were discussed, ensuring the participation and
cooperation of the PTA members, Teachers and the Stakeholders.
B. A general assembly was conducted on January 31, 2023 to inform all parents, teachers,
and students about the King and Queen of Heart fundraising program. The purpose, rules,
and benefits of the program were explained, encouraging active involvement and support
from the school community.
C. During the first canvassing, each learner in every class was given the opportunity to
contribute towards the fundraising goal. The student who made the highest donation in
each grade level was declared as the official candidate representing their respective
class, advancing to the next stage.
D. The official candidates from each grade level proceeded to the second canvassing. They
actively campaigned and solicited donations from their classmates, teachers, and family
members. The candidates utilized various fundraising strategies, such as organizing
events, conducting personal appeals, and engaging in creative initiatives.
E. The program culminated in the coronation ceremony, held to celebrate the efforts and
achievements of the candidates. The event showcased performances, speeches, and the
announcement of the winners for the King and Queen of Heart title. The coronation
ceremony was attended by students, parents, teachers, and members of the PTA.
F. Following the success of the fundraising program, the PTA utilized the funds raised to
purchase amplifier for the school. Careful consideration was given to selecting a reliable
supplier and acquiring high-quality equipment that met the school's requirements.
G. After the completion of the King and Queen of Heart program and the procurement of the
amplifier, the PTA conducted feedback sessions to gather input and suggestions from the
school community. This feedback aimed to assess the effectiveness of the program,
identify areas for improvement, and determine the satisfaction levels of the participants.
