DOCUMENT CHECKLIST
Include the following documents in your application package
1. Complete your citizenship card application and have two citizenship card photos taken (photo specification page included in this package). 2. Obtain notarized copies of both the applicant's birth certificate (showing the full names of the parents) and a document proving parent held Canadian citizenship at the time of applicant's birth (i.e. birth certificate, citizenship card, passport etc.) 2. Fax us your citizenship card application, notarized copies of two (2) pieces of identification (one of which MUST be photo identification), a travel booking (if you are traveling soon), and this order form to 416-640-2650. 3. We will call you to confirm the receipt of your fax and discuss your case. 4. Once we have instructed you to do so, courier your entire application package to us at: 1180 DANFORTH AVE, TORONTO, ON M4J 1M3, CANADA 5. We will contact you when your citizenship card is ready. Free pick-up is available in Toronto; otherwise we will courier the document to you. Courier charge: $26 within Canada APPLICATIONS MISSING THIS ORDER FORM WILL NOT BE PROCESSED.
