Adherence and Skills Assessment
This toolkit provides documents for assessing whether staff are adhering to the decolonization protocol. These documents include self-administered survey questions for patients to describe their bathing experience, as well as staff skills assessment questions for peer or supervisor observation of staff bathing practices.
Assessment documents are provided for each alternative approach to implementing decolonization. Hospitals should use the assessment documents that are appropriate to the protocol option that they choose to implement.
Assessment documents in this toolkit are provided in PDF versions as well as editable versions that allow hospitals to customize documents to meet their needs.
