Change of Location for a Driving Safety Provider

Give Advance Notice
Not less than 15 business days before your move, you must notify TDLR of your intent to move a driving safety school.
Application and Fee
You must submit a completed Driving Safety Provider Change of Address Form along with the non-refundable application fee of $50.
Surety Bond
One of the following must also be submitted along with the application:
- Original Bond or Alternate Form of Security - (see section 1001.209 for bond requirements)
- A rider from the surety bond company showing the new address listed
