HealthPartners offers advisors through FamilyPath. Call the number on the back of your medical insurance ID card to get connected. You may also call FamilyPath directly at 844-303-8455. FamilyPath advisors can answer fertility related questions and guide you through the process. They specialize in: 

• Matching you with a provider
• Helping you understand fertility treatments and what options are available through your Minnesota Advantage Health Plan coverage.
• Connecting you to other services that will help support you.

Your must get your care through a provider in your Blue Cross and Blue Shield of Minnesota or HealthPartners network.

If the provider you see is not in your network, you do not have coverage. Insurance will not pay the medical bill. You are responsible for paying the bill.

Treatment
Treatments can either attempt to restore fertility through medication or surgery, or help you get pregnant through procedures using assisted reproductive technology. You do not need to have an infertility diagnosis to use the treatment benefit. Covered treatments include:

• Artificial insemination
• Intrauterine insemination
• Assisted reproductive technology (ART) procedures such as
  • In-vitro fertilization
  • Gamete intrafallopian tube transfer
  • Zygote intrafallopian transfer
  • Intracytoplasmic sperm injection

Coverage also includes:

• Freezing (cryopreservation), thawing
• Embryo storage for up to one year

Mental health care you need related to fertility treatment is covered as all other mental health care. The cost of mental health care is not included in the $30,000 limit. In 2024, the copay and deductible for allowed office visits is $0 if your primary care clinic is a cost level 1 or 2. If your PCC in cost level 3 or 4 you will save $20 on allowed office visit copays, however the deductible is still in place.

Your prescription drug coverage through CVS Caremark helps pay for medications. Medications to treat infertility have the same monthly copay as any other medications. Medication expenses are separate from the $30,000 coverage limit. There is no dollar limit on medications.

If you need to travel more than 100 miles from your home to receive fertility care, you may use the travel benefit included in your medical plan. It will help pay for transportation and lodging. 

Call the member services on the back of your Blue Cross or HealthPartners ID card to get details. The travel benefit has a $5,000 lifetime maximum per person in the Minnesota Advantage Health Plan. It is not included in the $30,000 fertility benefit lifetime cap.

• Reversal of sterilization
• Fertility treatment after reversal of sterilization
• Sperm and ova storage
• Embryo storage after one year
• Surrogacy/gestational carrier compensation, services, and fees
• Procurement and fees associated with donor sperm and ova
• Maternity services for a surrogate/gestational carrier

• The most the plan will pay is $30,000. The $30,000 is a lifetime maximum.
• The cap excludes prescription drugs, mental health, and travel. That care is covered elsewhere in the plan.
• The lifetime maximum is combined between Blue Cross and HealthPartners. It doesn’t start over if you switch health plan administrators.
• You do not need an infertility diagnosis to use the benefit.
