The Transition Roadmaps were developed to serve as an important resource for transition-age students (age 14-21) and their families as they prepare for life after high school.
There are four roadmaps in total:
Candace Lindow-Davies and Kathy Manlapas, with support from their workgroup colleagues, developed the roadmap as a part of their participation in the Collaborative Plan's Transition workgroup goals. Luke Davies interviewed peers to provide relevant content for the roadmaps.
