Executive Order 96-9 requires State agencies and MnSCU to document compliance with Title I and II of the Americans with Disabilities Act, and submit an annual report to the Commissioner, Agency Head, Chancellor or President, and the State ADA Coordinator. The reporting period is from July 1st through June 30th. The report must be submitted online by September 1.
When completing the form, answer the questions regarding your agency, college, university, or system office's efforts toward increasing awareness and accessibility for people with disabilities. An Annual ADA Reporting Spreadsheet (excel version) has been created to help you collect the information for the report.
